Sub acute care is designed to treat patients who are recovering
from an acute illness, injury or debilitating medical condition,
and need extensive rehabilitation services and/or medical
treatment. It is more intensive than traditional nursing home
care. It is a goal-oriented treatment implemented immediately
following or instead of acute hospitalization. Sub acute care
is a specifically defined program where the primary goal is
to have the patient return to previous functionality as quickly
as possible.
Network facilities are staffed by doctors, physical therapists,
nurses and a wide array of specialists who utilize state-of-the-art
diagnostic and treatment technologies. Our network has the
resources to provide both the technology and human care for
our valued patients
Admissions
An admission into any health care center can often be an emotional
and difficult time. Our staff always makes itself available
in making this transition a pleasant one for everyone involved.
We want to assure a comfortable stay whether it is within
our skilled nursing unit, subacute unit, dialysis center or
outpatient therapy center. We are dedicated to serving individuals
who have been disabled due to illness or injury….the next
generation in healthcare….our facilities provide innovative
high quality, cost - effective health care programs and services
to meet the emerging health needs of the community.
We accept admissions 24 hours a day to accommodate you and
your needs. Referrals to the center can be made by contacting
the Admissions Department. We welcome referrals from social
workers, discharge planners, case managers, physicians, insurance
representatives and families.
At the end of the day, our company commitment is to those
who are in our care. Our skilled and intermediate nursing
facilities provide a high level of nursing and other medical
care, as well as personal care and assistance, for people
whose illness or impairments require close monitoring.
Around the clock nursing is available with registered nurses
on duty at all times. In addition to nursing, other medical
services are provided including various rehabilitative therapies:

- Post-hospital and post-surgical care

American Health Care Association Quality Award
Foresight managed facilities have been recipients of five Quality Awards from the American Health Care Association. San Bruno Skilled Nursing Hospital, Woodland Nursing Inn, and McClure Rehabilitation Center were Foresight’s initial recipients of Step I awards in 1996. One year later, these facilities became three of the first four recipients of the Step 2 Quality Award.
In 2004, Foresight managed facilities returned to receive national recognition with Brookside Skilled Nursing Hospital and the Reutlinger Community for Jewish Living receiving Step I awards.
